In response to the current housing crisis, we have designed two new pathways for our customers/clients.
Wollongong Homeless Hub is known for its innovation and is unique in its flexible service delivery and approach to filling the Gaps that often present for our Homeless community members.
We have found the need to find ways to stay connected and support our rough sleepers to stay safe and healthy again providing us an opportunity to think outside of the box. Our new Wollongong Homeless Hub App compliments our assertive outreach program named “Connection Point” aimed at connecting and locating rough sleepers that may be falling through the local service Gaps.
Unlike City areas our homeless community are not all located in one area and often are spread over varies suburbs making traditional assertive outreach programs difficult to deliver to the target group.
By downloading the App and clicking on the unique mapping feature community members are asked to participate in connecting our vulnerable community members by dropping a Virtual Pin on the map which will provide an outreach point to for our team to deliver face to face contact with the person sleeping rough.
Once connected with the person they will receive up to date advice and information. The opportunity to be assessed for crisis accommodation and referral pathways for health and any other identified barriers that may be preventing the person from transitioning from “homelessness to home”.
